Mayhew Consultations, LLC will protect your personal information and not sell this to any vendors.
For all AHA classes, registration information is submitted to an AHA Training Center. Certification cards are issued by the Training Center.
For MAT, MAT IS and PMAT, registration information is submitted to VDSS.
All rosters and tests are maintained by Mayhew Consultations, LLC in strict privacy with limited access by personnel.
Physical or electronic access to devices containing personal or class related information is limited strictly to employees, authorized subcontractors and technicians of or on the behalf of Mayhew Consultations, LLC.
All courses are taught in English only. All videos and written instructions are in English only.
MAT, MAT IS and PMAT require proof of current CPR & First Aid prior to date of class. All participants must be at least 18 years old, be affiliated with a child day program and be literate (read and write) in the language the healthcare provider instructions and parental permissions are written. MAT, MAT IS and PMAT require photo ID at registration the day of class. No money will be refunded for class and you will not participate in class without a photo ID.
Only participants may attend classes. Children must be 14 years or older to participate in CPR.
Payment in full is required before class. No refund for cancellations less than twenty-four hours. If instructor cancels class due to weather, illness or other unforeseen events, class will be rescheduled. If class is not rescheduled, payment will be refunded. If class is cancelled due to lack of participants, payment will be refunded.
